accept the invitation to participate in the Underclassman All-American Select Game to be held in Atlanta on Sunday September 1st 2025 and I will abide by all rules established for participants. I understand that my participation in the game will include physical activities and I represent that I have been examined by a physician within the past year and have been advised that I am fit and capable of handling the rigors of the activities on this weekend. In consideration of my acceptance to the game, I on behalf of myself, my heirs, executors and administrators waive and release the Underclassman All-American Game (UCAAG), all employees/volunteers, agents, representatives and coaches of the UCAAG, As well as Overtime Elite (OTE) and Holy Innocent Episcopal High School, their respective employees, agents, volunteers and representatives from any and all rights and claims for damages I may incur arising out of injury or illness suffered by me as a result of my participation or attendance in the Underclassman AllAmerican Game - Hoops for Diabetes.
